A questionnaire is a research instrument consisting of a series of questions for the purpose of gathering information from respondents. Questionnaires can be thought of as a kind of written interview . They can be carried out face to face, by telephone, computer, or post.

Questionnaires provide a relatively cheap, quick, and efficient way of obtaining large amounts of information from a large sample of people.

Questionnaire

Data can be collected relatively quickly because the researcher would not need to be present when completing the questionnaires. This is useful for large populations when interviews would be impractical.

However, a problem with questionnaires is that respondents may lie due to social desirability. Most people want to present a positive image of themselves, and may lie or bend the truth to look good, e.g., pupils exaggerate revision duration.

Questionnaires can effectively measure relatively large subjects’ behavior, attitudes, preferences, opinions, and intentions more cheaply and quickly than other methods.

Often, a questionnaire uses both open and closed questions to collect data. This is beneficial as it means both quantitative and qualitative data can be obtained.

Closed Questions

A closed-ended question requires a specific, limited response, often “yes” or “no” or a choice that fit into pre-decided categories.

Data that can be placed into a category is called nominal data. The category can be restricted to as few as two options, i.e., dichotomous (e.g., “yes” or “no,” “male” or “female”), or include quite complex lists of alternatives from which the respondent can choose (e.g., polytomous).

Closed questions can also provide ordinal data (which can be ranked). This often involves using a continuous rating scale to measure the strength of attitudes or emotions.

For example, strongly agree / agree / neutral / disagree / strongly disagree / unable to answer.

Closed questions have been used to research type A personality (e.g., Friedman & Rosenman, 1974) and also to assess life events that may cause stress (Holmes & Rahe, 1967) and attachment (Fraley, Waller, & Brennan, 2000).

  • They can be economical. This means they can provide large amounts of research data for relatively low costs. Therefore, a large sample size can be obtained, which should represent the population from which a researcher can then generalize.
  • The respondent provides information that can be easily converted into quantitative data (e.g., count the number of “yes” or “no” answers), allowing statistical analysis of the responses.
  • The questions are standardized. All respondents are asked exactly the same questions in the same order. This means a questionnaire can be replicated easily to check for reliability . Therefore, a second researcher can use the questionnaire to confirm consistent results.

Limitations

  • They lack detail. Because the responses are fixed, there is less scope for respondents to supply answers that reflect their true feelings on a topic.

Open Questions

Open questions allow for expansive, varied answers without preset options or limitations.

Open questions allow people to express what they think in their own words. Open-ended questions enable the respondent to answer in as much detail as they like in their own words. For example: “can you tell me how happy you feel right now?”

Open questions will work better if you want to gather more in-depth answers from your respondents. These give no pre-set answer options and instead, allow the respondents to put down exactly what they like in their own words.

Open questions are often used for complex questions that cannot be answered in a few simple categories but require more detail and discussion.

Lawrence Kohlberg presented his participants with moral dilemmas. One of the most famous concerns a character called Heinz, who is faced with the choice between watching his wife die of cancer or stealing the only drug that could help her.

Participants were asked whether Heinz should steal the drug or not and, more importantly, for their reasons why upholding or breaking the law is right.

  • Rich qualitative data is obtained as open questions allow respondents to elaborate on their answers. This means the research can determine why a person holds a certain attitude .
  • Time-consuming to collect the data. It takes longer for the respondent to complete open questions. This is a problem as a smaller sample size may be obtained.
  • Time-consuming to analyze the data. It takes longer for the researcher to analyze qualitative data as they have to read the answers and try to put them into categories by coding, which is often subjective and difficult. However, Smith (1992) has devoted an entire book to the issues of thematic content analysis that includes 14 different scoring systems for open-ended questions.
  • Not suitable for less educated respondents as open questions require superior writing skills and a better ability to express one’s feelings verbally.

Questionnaire Design

With some questionnaires suffering from a response rate as low as 5%, a questionnaire must be well designed.

There are several important factors in questionnaire design.

Pilot Study

Question order.

Questions should progress logically from the least sensitive to the most sensitive, from the factual and behavioral to the cognitive, and from the more general to the more specific.

The researcher should ensure that previous questions do not influence the answer to a question.

Question order effects

  • Question order effects occur when responses to an earlier question affect responses to a later question in a survey. They can arise at different stages of the survey response process – interpretation, information retrieval, judgment/estimation, and reporting.
  • Types of question order effects include: unconditional (subsequent answers affected by prior question topic), conditional (subsequent answers depend on the response to the prior question), and associational (correlation between two questions changes based on order).
  • Question order effects have been found across different survey topics like social and political attitudes, health and safety studies, vignette research, etc. Effects may be moderated by respondent factors like age, education level, knowledge and attitudes about the topic.
  • To minimize question order effects, recommendations include avoiding judgmental dependencies, separating potentially reactive questions, randomizing questions, following good survey design principles, considering respondent characteristics, and intentionally examining question context and order.

Terminology

  • There should be a minimum of technical jargon. Questions should be simple, to the point, and easy to understand. The language of a questionnaire should be appropriate to the vocabulary of the group of people being studied.
  • Use statements that are interpreted in the same way by members of different subpopulations of the population of interest.
  • For example, the researcher must change the language of questions to match the social background of the respondent’s age / educational level / social class/ethnicity, etc.

  • The researcher must ensure that the information provided by the respondent is kept confidential, e.g., name, address, etc.
  • This means questionnaires are good for researching sensitive topics as respondents will be more honest when they cannot be identified.
  • Keeping the questionnaire confidential should also reduce the likelihood of psychological harm, such as embarrassment.
  • Participants must provide informed consent before completing the questionnaire and must be aware that they have the right to withdraw their information at any time during the survey/ study.

Problems with Postal Questionnaires

At first sight, the postal questionnaire seems to offer the opportunity to get around the problem of interview bias by reducing the personal involvement of the researcher. Its other practical advantages are that it is cheaper than face-to-face interviews and can quickly contact many respondents scattered over a wide area.

However, these advantages must be weighed against the practical problems of conducting research by post. A lack of involvement by the researcher means there is little control over the information-gathering process.

The data might not be valid (i.e., truthful) as we can never be sure that the questionnaire was completed by the person to whom it was addressed.

That, of course, assumes there is a reply in the first place, and one of the most intractable problems of mailed questionnaires is a low response rate. This diminishes the reliability of the data

Also, postal questionnaires may not represent the population they are studying. This may be because:

  • Some questionnaires may be lost in the post, reducing the sample size.
  • The questionnaire may be completed by someone not a member of the research population.
  • Those with strong views on the questionnaire’s subject are more likely to complete it than those without interest.

Benefits of a Pilot Study

A pilot study is a practice / small-scale study conducted before the main study.

It allows the researcher to try out the study with a few participants so that adjustments can be made before the main study, saving time and money.

It is important to conduct a questionnaire pilot study for the following reasons:

  • Check that respondents understand the terminology used in the questionnaire.
  • Check that emotive questions are not used, as they make people defensive and could invalidate their answers.
  • Check that leading questions have not been used as they could bias the respondent’s answer.
  • Ensure the questionnaire can be completed in an appropriate time frame (i.e., it’s not too long).

You don’t have to be an expert to create a survey, but by following a few survey best practices you can make sure you’re collecting the best data possible.

From working out what you want to achieve to providing incentives for respondents, survey design can take time.

But when you don’t have hours to devote to becoming a survey-creation guru, a quick guide to the essentials is a great way to get started.

In this article, we’re going to reveal how to create a survey that’s easy to complete, encourages collecting feedback, hits the research questions you’re interested in, and produces data that’s easy to work with at the analysis stage .

15 Tips when creating surveys

1. define the purpose of the survey.

Before you even think about your survey questions , you need to define their purpose.

The survey’s purpose should be a clear, attainable, and relevant goal. For example, you might want to understand why customer engagement is dropping off during the middle of the sales process.

Your goal could then be something like: “I want to understand the key factors that cause engagement to dip at the middle of the sales process, including both internal and external elements.”

Or maybe you want to understand customer satisfaction post-sale. If so, the goal of your survey could be: “I want to understand how customer satisfaction is influenced by customer service and support post-sale, including through online and offline channels.”

The idea is to come up with a specific, measurable, and relevant goal for your survey. This way you ensure that your questions are tailored to what you want to achieve and that the data captured can be compared against your goal.

2. Make every question count

You’re building your survey questionnaire to obtain important insights, so every question should play a direct role in hitting that target.

Make sure each question adds value and drives survey responses that relate directly to your research goals. For example, if your participant’s precise age or home state is relevant to your results, go ahead and ask. If not, save yourself and your respondents some time and skip it.

It’s best to plan your survey by first identifying the data you need to collect and then writing your questions.

You can also incorporate multiple-choice questions to get a range of responses that provide more detail than a solid yes or no. It’s not always black and white.

3. Keep it short and simple

Although you may be deeply committed to your survey, the chances are that your respondents... aren’t.

As a survey designer, a big part of your job is keeping their attention and making sure they stay focused until the end of the survey.

Respondents are less likely to complete long surveys or surveys that bounce around haphazardly from topic to topic. Make sure your survey follows a logical order and takes a reasonable amount of time to complete.

Although they don’t need to know everything about your research project, it can help to let respondents know why you’re asking about a certain topic. Knowing the basics about who you are and what you’re researching means they’re more likely to keep their responses focused and in scope.

4. Ask direct questions

Vaguely worded survey questions confuse respondents and make your resulting data less useful. Be as specific as possible, and strive for clear and precise language that will make your survey questions easy to answer.

It can be helpful to mention a specific situation or behavior rather than a general tendency. That way you focus the respondent on the facts of their life rather than asking them to consider abstract beliefs or ideas .

5. Ask one question at a time

Although it’s important to keep your survey as short and sweet as possible, that doesn’t mean doubling up on questions. Trying to pack too much into a single question can lead to confusion and inaccuracies in the responses.

Take a closer look at questions in your survey that contain the word “and” – it can be a red flag that your question has two parts. For example: “Which of these cell phone service providers has the best customer support and reliability?” This is problematic because a respondent may feel that one service is more reliable, but another has better customer support.

6. Avoid leading and biased questions

Although you don’t intend them to, certain words and phrases can introduce bias into your questions or point the respondent in the direction of a particular answer.

As a rule of thumb, when you conduct a survey it’s best to provide only as much wording as a respondent needs to give an informed answer. Keep your question wording focused on the respondent and their opinions, rather than introducing anything that could be construed as a point of view of your own.

In particular, scrutinize adjectives and adverbs in your questions. If they’re not needed, take them out.

7. Speak your respondent's language

This tip goes hand in hand with many others in this guide – it’s about making language only as complex or as detailed as it needs to be when conducting great surveys.

Create surveys that use language and terminology that your respondents will understand. Keep the language as plain as possible, avoid technical jargon and keep sentences short. However, beware of oversimplifying a question to the point that its meaning changes.

8. Use response scales whenever possible

Response scales capture the direction and intensity of attitudes, providing rich data. In contrast, categorical or binary response options, such as true/false or yes/no response options, generally produce less informative data.

If you’re in the position of choosing between the two, the response scale is likely to be the better option.

Avoid using scales that ask your target audience to agree or disagree with statements, however. Some people are biased toward agreeing with statements , and this can result in invalid and unreliable data.

9. Avoid using grids or matrices for responses

Grids or matrices of answers demand a lot more thinking from your respondent than a scale or multiple choice question. They need to understand and weigh up multiple items at once, and oftentimes they don’t fill in grids accurately or according to their true feelings .

Another pitfall to be aware of is that grid question types aren’t mobile-friendly. It’s better to separate questions with grid responses into multiple questions in your survey with a different structure such as a response scale.

10. Rephrase yes/no questions if possible in online survyes

As we’ve described, yes/no questions provide less detailed data than a response scale or multiple-choice, since they only yield one of two possible answers.

Many yes/no questions can be reworked by including phrases such as “How much,” “How often,” or “How likely.” Make this change whenever possible and include a response scale for richer data.

By rephrasing your questions in this way, your survey results will be far more comprehensive and representative of how your respondents feel.

11. Start with the straightforward stuff

Ease your respondent into the survey by asking easy questions at the start of your questionnaire, then moving on to more complex or thought-provoking elements once they’re engaged in the process.

This is especially valuable if you need to cover any potentially sensitive topics in your survey. Never put sensitive questions at the start of the questionnaire where they’re more likely to feel off-putting.

Your respondent will probably become more prone to fatigue and distraction towards the end of the survey, so keep your most complex or contentious questions in the middle of the survey flow rather than saving them until last.

12. Use unbalanced scales with care

Unbalanced response scales and poorly worded questions can mislead respondents.

For example, if you’ve asked them to rate a product or service and you provide a scale that includes “poor”, “satisfactory”, “good” and “excellent”, they could be swayed towards the “excellent” end of the scale because there are more positive options available.

Make sure your response scales have a definitive, neutral midpoint (aim for odd numbers of possible responses) and that they cover the whole range of possible reactions to the question .

13. Consider adding incentives

To increase the number of responses, incentives — discounts, offers, gift cards, or sweepstakes — can prove helpful.

Of course, while the benefits of offering incentives sound appealing (more respondents), there’s the possibility of attracting the opinions of the wrong audiences, such as those who are only in it for the incentive.

With this in mind, make sure you limit your surveys to your target population and carefully assess which incentives would be most valuable to them.

However short or straightforward your questionnaire is, it’s always a good idea to pre-test your survey before you roll it out fully so that you can catch any possible errors before they have a chance to mess up your survey results.

Share your survey with at least five people, so that they can test your survey to help you catch and correct problems before you distribute it.

How to Make a Questionnaire

When a company, non-profit group, or politician needs to find out how their stakeholders or constituents feel, they often create and implement a questionnaire. The results can lead to re-branding, decision-making, and policy changes if the feedback is sound. Making a questionnaire can seem very straightforward, but unless it is designed properly, the results can be skewed and unreliable.

Creating Questions

Step 1 Decide what you want to learn from administering your questionnaire.

Tip: Ideally, the questionnaire will be short, so decide which of your goals are essential and which might be unnecessary.

Step 2 Plan questions that will help you get the information you need.

  • "Have you shopped here before?"
  • "If so, how often do you shop here?" (This question would have a few explicit answers from which respondents could choose--"once a week" to "once a month," for instance)
  • "How satisfied were you with your experience today?" (Likewise, this question would have limited responses--"very satisfied" to "very dissatisfied")
  • "Would you recommend this store to a friend?"

Step 4 Use open-ended questions to solicit feedback.

  • "How will you use your purchase?"
  • "Where else do you normally shop?"
  • "Who referred you to this store?"
  • Open-ended questions are good for clarifying a previous answer--"Why do you feel this way?"

Step 5 Ask questions in such a way as to avoid confusion and bias.

  • Questions should be worded so as to maximize clarity. Confused respondents will skew your data, so questions should be as understandable as possible. Avoid double negatives, unnecessary clauses, or unclear subject-object relationships.

Implementing the Questionnaire

Step 1 Think about how you will deliver your questionnaire.

  • Surveys delivered on the computer, by phone, and by mail can reach a broad range of people, whereas surveys administered in-person are time-intensive and limits who can participate (which may be useful).
  • Surveys delivered on the computer, in person, and by mail can utilize pictures, whereas phone interviews cannot.
  • Respondents may be too shy to answer certain questions in person or by phone. Decide if you want to give clarifications to your questions if the respondent doesn't understand something; only interviews given by a live person can deliver clarifications.
  • A computer survey will require the respondent to have access to a computer. If your questionnaire concerns private issues, a computer survey may work best. [4] X Research source

Step 3 Consider the order of your questions.

  • "Qualifiers" are questions that screen certain respondents out, preventing them from completing other questions. Position these at the beginning of your questionnaire.
  • If demographics are of major concern, ask demographic questions up front.
  • Save personal or complicated questions for the end of the questionnaire. Respondents will not feel as overwhelmed by these questions and may be more likely to be open and honest.

Note: You may want to order the questions so that if a person says yes or no to a certain question, they bypass any questions that don't apply to them. This will help keep the questionnaire focused and take less time to complete.

Step 4 Decide if you will offer incentives for completing the questionnaire.

  • Ask your testers for feedback. They may alert you to sections that confused them or felt out of place. User impressions about the questionnaire are just as important as the actual questionnaire.
  • After you test, do some number crunching to ensure you are collecting the data you need. If you are not getting the information you want, adjust the questionnaire. You may need to reword some things, add introductions, or rearrange, add, or delete questions so your questionnaire leads you toward your goals.

Revising the Questionnaire

Step 1 Review your data to understand what your questionnaire was really asking.

  • For instance, you may find that a question such as "How often do you shop here?" limits your demographic to those who shop at a brick-and-mortar store. If you want to see how people purchase a specific product, you may want to broaden your question to include online shopping.
  • Your implementation method may also be limiting your data. For instance, surveys administered online may be answered largely by respondents with higher-than-average computer knowledge.

Step 2 Further revise your questions.

  • For instance, a question such as, "Why do you shop here?" may be too broad a question, which could mislead your respondents. If you want to know if the store's decor has an impact on shopping habits, you could instead ask respondents to describe how they feel about the store's decor, branding, etc.

Step 3 Review your open-ended questions.

  • As above, broad questions such as, "How do you feel while shopping here?" may not give your respondents enough direction. You could instead ask, "Would you recommend this store to your friends? Why or why not?"

Step 4 Decide how you will respond to missing data.

  • For instance, if you are asking respondents to rate an experience, you should provide them with the option to respond with "very dissatisfied" as well as "very satisfied," and many options in between.

How to conduct your own market research survey (with example)

After watching a few of those sketches, you can imagine why real-life focus groups tend to be pretty small. Even without any over-the-top personalities involved, it's easy for these groups to go off the rails.

So what happens when you want to collect market research at a larger scale? That's where the market research survey comes in. Market surveys allow you to get just as much valuable information as an in-person interview, without the burden of herding hundreds of rowdy Eagles fans through a product test.

What is a market research survey?

Market surveys are what's known as "primary research"—that is, information that the researching company gathers firsthand. Secondary research consists of data that another organization gathered and published, which other researchers can then use for their own reports. Primary research is more expensive and time-intensive than secondary research, which is why you should only use market research surveys to obtain information that you can't get anywhere else. 

A market research survey can collect information on your target customers':

Experiences

Preferences, desires, and needs

Values and motivations

The types of information that can usually be found in a secondary source, and therefore aren't good candidates for a market survey, include your target customers':

Demographic data

Consumer spending data

Household size

Why conduct market research?

Here are some examples of how market research surveys can be used to fill a wide range of knowledge gaps for companies:

A B2B software company asks real users in its industry about Kanban board usage to help prioritize their project view change rollout.

A B2C software company asks its target demographic about their mobile browsing habits to help them find features to incorporate into their forthcoming mobile app.

A printing company asks its target demographic about fabric preferences to gauge interest in a premium material option for their apparel lines.

A wholesale food vendor surveys regional restaurant owners to find ideas for seasonal products to offer.

Primary vs. secondary market research

Market surveys are what's known as "primary research"—that is, information that the researching company gathers firsthand. Secondary research consists of data that another organization gathered and published, which other researchers can then use for their own reports. 

Primary research is more expensive and time-intensive than secondary research, which is why you should only use market research surveys to obtain information that you can't get anywhere else. 

If you've exhausted your secondary research options and still have unanswered questions, it's time to start thinking about conducting a market research survey.

6 types of market research survey

Depending on your goal, you'll need different types of market research. Here are six types of market research surveys.

1. Buyer persona research

A buyer persona research survey will help you learn more about things like demographics, household makeup, income and education levels, and lifestyle markers. The more you learn about your existing customers, the more specific you can get in targeting potential customers. You may find that there are more buyer personas within your user base than the ones that you've been targeting.

2. Sales funnel research

With a sales funnel research survey, you can learn about potential customers' main drivers at different stages of the sales funnel. You can also get feedback on how effective different sales strategies are. Use this survey to find out:

How close potential buyers are to making a purchase

What tools and experiences have been most effective in moving prospective customers closer to conversion

3. Customer loyalty research

The demographics of your most loyal customers

What tools are most effective in turning customers into advocates

What you can do to encourage more brand loyalty

4. Branding and marketing research

The Charmin focus group featured in that SNL sketch is an example of branding and marketing research, in which a company looks for feedback on a particular advertising angle to get a sense of whether it will be effective before the company spends money on running the ad at scale. Use this type of survey to find out:

Whether a new advertising angle will do well with existing customers

Whether a campaign will do well with a new customer segment you haven't targeted yet

What types of campaign angles do well with a particular demographic

5. New products or features research

What features they wish your product currently had

What they think of a particular product or feature idea

6. Competitor research

Whether your competitors have found success with a buyer persona you're not targeting

Information about buyers for a product that's similar to one you're thinking about launching

Feedback on what features your competitors' customers wish their version of a product had

How to write and conduct a market research survey

Once you've narrowed down your survey's objectives, you can move forward with designing and running your survey.

Step 1: Write your survey questions

A poorly worded survey, or a survey that uses the wrong question format, can render all of your data moot. If you write a question that results in most respondents answering "none of the above," you haven't learned much. 

Categorical questions

Also known as a nominal question, this question type provides numbers and percentages for easy visualization, like "35% said ABC." It works great for bar graphs and pie charts, but you can't take averages or test correlations with nominal-level data.

Multiple choice: Use this type of question if you need more nuance than a Yes/No answer gives. You can add as many answers as you want, and your respondents can pick only one answer to the question. 

Checkbox: Checkbox questions add the flexibility to select all the answers that apply. Add as many answers as you want, and respondents aren't limited to just one.

Ordinal questions

This type of question requires survey-takers to pick from options presented in a specific order, like "income of $0-$25K, $26K-$40K, $41K+." Like nominal questions, ordinal questions elicit responses that allow you to analyze counts and percentages, though you can't calculate averages or assess correlations with ordinal-level data.

Dropdown: Responses to ordinal questions can be presented as a dropdown, from which survey-takers can only make one selection. You could use this question type to gather demographic data, like the respondent's country or state of residence. 

Ranking: This is a unique question type that allows respondents to arrange a list of answers in their preferred order, providing feedback on each option in the process. 

Interval/ratio questions

For precise data and advanced analysis, use interval or ratio questions. These can help you calculate more advanced analytics, like averages, test correlations, and run regression models. Interval questions commonly use scales of 1-5 or 1-7, like "Strongly disagree" to "Strongly agree." Ratio questions have a true zero and often ask for numerical inputs (like "How many cups of coffee do you drink per day? ____").

Ranking scale: A ranking scale presents answer choices along an ordered value-based sequence, either using numbers, a like/love scale, a never/always scale, or some other ratio interval. It gives more insight into people's thoughts than a Yes/No question. 

Matrix: Have a lot of interval questions to ask? You can put a number of questions in a list and use the same scale for all of them. It simplifies gathering data about a lot of similar items at once. 

Example : How much do you like the following: oranges, apples, grapes? Hate/Dislike/Ok/Like/Love

Textbox: A textbox question is needed for collecting direct feedback or personal data like names. There will be a blank space where the respondent can enter their answer to your question on their own.

Step 2: Choose a survey platform

Most survey apps today look great on mobile, but be sure to preview your survey on your phone and computer, at least, to make sure it'll look good for all of your users.

If you have the budget, you can also purchase survey services from a larger research agency. 

Step 3: Run a test survey

Before you run your full survey, conduct a smaller test on 5%-10% of your target respondent pool size. This will allow you to work out any confusing wording or questions that result in unhelpful responses without spending the full cost of the survey. Look out for:

Survey rejection from the platform for prohibited topics

Joke or nonsense textbox answers that indicate the respondent didn't answer the survey in earnest

Multiple choice questions with an outsized percentage of "none of the above" or "N/A" responses

Step 4: Launch your survey

If your test survey comes back looking good, you're ready to launch the full thing! Make sure that you leave ample time for the survey to run—you'd be surprised at how long it takes to get a few thousand respondents. 

Even if you've run similar surveys in the past, leave more time than you need. Some surveys take longer than others for no clear reason, and you also want to build in time to conduct a comprehensive data analysis.

Step 5: Organize and interpret the data

Tips for running a market research survey.

You know the basics of how to conduct a market research survey, but here are some tips to enhance the quality of your data and the reliability of your findings.

Find the right audience: You could have meticulously crafted survey questions, but if you don't target the appropriate demographic or customer segment, it doesn't really matter. You need to collect responses from the people you're trying to understand. Targeted audiences you can send surveys to include your existing customers, current social media followers, newsletter subscribers, attendees at relevant industry events, and community members from online forums, discussion boards, or other online communities that cater to your target audience. 

Focus questions on a desired data type: As you conceptualize your survey, consider whether a qualitative or quantitative approach will better suit your research goals. Qualitative methods are best for exploring in-depth insights and underlying motivations, while quantitative methods are better for obtaining statistical data and measurable trends. For an outcome like "optimize our ice cream shop's menu offerings," you may want to find out which flavors of ice cream are most popular with teens. This would require a quantitative approach, for which you would use categorical questions that can help you rank potential flavors numerically.

Establish a timeline: Set a realistic timeline for your survey, from creation to distribution to data collection and analysis. You'll want to balance having your survey out long enough to generate a significant amount of responses but not so long that it loses relevance. That length can vary widely based on factors like type of survey, number of questions, audience size, time sensitivity, question format, and question length.

Let's say you own a market research company, and you want to use a survey to gain critical insights into your market. You prompt users to fill out your survey before they can access gated premium content.

Survey findings: 

63% of respondents represent businesses with over 100 employees, while only 8% represent businesses with under 10.

71% of respondents work in sales, marketing, or operations.

80% of respondents consider access to market data to be either very important or critically important.

"Timeliness of data" (38%) and "Accuracy of data" (32%) were most commonly ranked as the most important market data access factor.

86% of respondents claimed that their jobs rely on accessing accurate, up-to-date market data.

Insights and recommendations: Independent analysis of the survey indicates that a large percentage of users work in the sales, marketing, or operations fields of large companies, and these customers value timeliness and accuracy most. These findings can help you position future report offerings more effectively by highlighting key benefits that are important to customers that fit into related customer profiles.

Data Centers Could Use 9% of US Electricity by 2030, Research Institute Says

NEW YORK (Reuters) - Data centers could use up to 9% of total electricity generated in the United States by the end of the decade, more than doubling their current consumption, as technology companies pour funds into expanding their computing hubs, the Electric Power Research Institute said on Wednesday.

   Depending on the adoption pace of technology such as generative artificial intelligence (AI), which is fueling the expansion of data centers, and the energy efficiency of new centers, the estimated annual growth rate of electricity use by the industry ranges from 3.7% to 15% through 2030, the institute's analysis said. The institute is a U.S.-based research organization funded by energy and government organizations.

WHY IT'S IMPORTANT

Data centers, along with expanding domestic manufacturing and electrification of transportation, are lifting the U.S. electricity industry out of two decades of flat growth.

The centers require massive amounts of power for high-intensity computing and cooling systems, with a new large data center requiring the same amount of electricity needed to power 750,000 homes, according to numerous energy company earnings calls this year.

A doubling in data centers' power use could strain the country's electric grid and lead to rising power bills and outages.

Since the roll-out of OpenAI's ChatGPT in 2022, the data center business has become one of the world's fastest-growing industries.

While early ChatGPT searches required about 10 times the electricity of a typical Google search, the growing use of generative AI to make movies and music could require vastly more power, the institute said.

"With 5.3 billion global internet users, widespread adoption of these tools could potentially lead to a step change in power requirements," according to the institute, which recommended better data center energy efficiency and more grid investment.

About 80% of the 2023 U.S. data center load was concentrated in 15 states, primarily Virginia and Texas, the institute said.

Questionnaire Design | Methods, Question Types & Examples

A questionnaire is a list of questions or items used to gather data from respondents about their attitudes, experiences, or opinions. Questionnaires can be used to collect quantitative and/or qualitative information.

Questionnaires are commonly used in market research as well as in the social and health sciences. For example, a company may ask for feedback about a recent customer service experience, or psychology researchers may investigate health risk perceptions using questionnaires.

A survey is a research method where you collect and analyse data from a group of people. A questionnaire is a specific tool or instrument for collecting the data.

Designing a questionnaire means creating valid and reliable questions that address your research objectives, placing them in a useful order, and selecting an appropriate method for administration.

But designing a questionnaire is only one component of survey research. Survey research also involves defining the population you’re interested in, choosing an appropriate sampling method , administering questionnaires, data cleaning and analysis, and interpretation.

Sampling is important in survey research because you’ll often aim to generalise your results to the population. Gather data from a sample that represents the range of views in the population for externally valid results. There will always be some differences between the population and the sample, but minimising these will help you avoid sampling bias .

Questionnaires can be self-administered or researcher-administered . Self-administered questionnaires are more common because they are easy to implement and inexpensive, but researcher-administered questionnaires allow deeper insights.

Self-administered questionnaires

Self-administered questionnaires can be delivered online or in paper-and-pen formats, in person or by post. All questions are standardised so that all respondents receive the same questions with identical wording.

Self-administered questionnaires can be:

  • Cost-effective
  • Easy to administer for small and large groups
  • Anonymous and suitable for sensitive topics

But they may also be:

  • Unsuitable for people with limited literacy or verbal skills
  • Susceptible to a nonreponse bias (most people invited may not complete the questionnaire)
  • Biased towards people who volunteer because impersonal survey requests often go ignored

Researcher-administered questionnaires

Researcher-administered questionnaires are interviews that take place by phone, in person, or online between researchers and respondents.

Researcher-administered questionnaires can:

  • Help you ensure the respondents are representative of your target audience
  • Allow clarifications of ambiguous or unclear questions and answers
  • Have high response rates because it’s harder to refuse an interview when personal attention is given to respondents

But researcher-administered questionnaires can be limiting in terms of resources. They are:

  • Costly and time-consuming to perform
  • More difficult to analyse if you have qualitative responses
  • Likely to contain experimenter bias or demand characteristics
  • Likely to encourage social desirability bias in responses because of a lack of anonymity

Your questionnaire can include open-ended or closed-ended questions, or a combination of both.

Using closed-ended questions limits your responses, while open-ended questions enable a broad range of answers. You’ll need to balance these considerations with your available time and resources.

Closed-ended questions

Closed-ended, or restricted-choice, questions offer respondents a fixed set of choices to select from. Closed-ended questions are best for collecting data on categorical or quantitative variables.

Categorical variables can be nominal or ordinal. Quantitative variables can be interval or ratio. Understanding the type of variable and level of measurement means you can perform appropriate statistical analyses for generalisable results.

Examples of closed-ended questions for different variables

Nominal variables include categories that can’t be ranked, such as race or ethnicity. This includes binary or dichotomous categories.

It’s best to include categories that cover all possible answers and are mutually exclusive. There should be no overlap between response items.

In binary or dichotomous questions, you’ll give respondents only two options to choose from.

White Black or African American American Indian or Alaska Native Asian Native Hawaiian or Other Pacific Islander

Ordinal variables include categories that can be ranked. Consider how wide or narrow a range you’ll include in your response items, and their relevance to your respondents.

Likert-type questions collect ordinal data using rating scales with five or seven points.

When you have four or more Likert-type questions, you can treat the composite data as quantitative data on an interval scale . Intelligence tests, psychological scales, and personality inventories use multiple Likert-type questions to collect interval data.

With interval or ratio data, you can apply strong statistical hypothesis tests to address your research aims.

Pros and cons of closed-ended questions

Well-designed closed-ended questions are easy to understand and can be answered quickly. However, you might still miss important answers that are relevant to respondents. An incomplete set of response items may force some respondents to pick the closest alternative to their true answer. These types of questions may also miss out on valuable detail.

To solve these problems, you can make questions partially closed-ended, and include an open-ended option where respondents can fill in their own answer.

Open-ended questions

Open-ended, or long-form, questions allow respondents to give answers in their own words. Because there are no restrictions on their choices, respondents can answer in ways that researchers may not have otherwise considered. For example, respondents may want to answer ‘multiracial’ for the question on race rather than selecting from a restricted list.

  • How do you feel about open science?
  • How would you describe your personality?
  • In your opinion, what is the biggest obstacle to productivity in remote work?

Open-ended questions have a few downsides.

They require more time and effort from respondents, which may deter them from completing the questionnaire.

For researchers, understanding and summarising responses to these questions can take a lot of time and resources. You’ll need to develop a systematic coding scheme to categorise answers, and you may also need to involve other researchers in data analysis for high reliability .

Question wording can influence your respondents’ answers, especially if the language is unclear, ambiguous, or biased. Good questions need to be understood by all respondents in the same way ( reliable ) and measure exactly what you’re interested in ( valid ).

Use clear language

You should design questions with your target audience in mind. Consider their familiarity with your questionnaire topics and language and tailor your questions to them.

For readability and clarity, avoid jargon or overly complex language. Don’t use double negatives because they can be harder to understand.

Use balanced framing

Respondents often answer in different ways depending on the question framing. Positive frames are interpreted as more neutral than negative frames and may encourage more socially desirable answers.

Use a mix of both positive and negative frames to avoid bias , and ensure that your question wording is balanced wherever possible.

Unbalanced questions focus on only one side of an argument. Respondents may be less likely to oppose the question if it is framed in a particular direction. It’s best practice to provide a counterargument within the question as well.

Avoid leading questions

Leading questions guide respondents towards answering in specific ways, even if that’s not how they truly feel, by explicitly or implicitly providing them with extra information.

It’s best to keep your questions short and specific to your topic of interest.

  • The average daily work commute in the US takes 54.2 minutes and costs $29 per day. Since 2020, working from home has saved many employees time and money. Do you favour flexible work-from-home policies even after it’s safe to return to offices?
  • Experts agree that a well-balanced diet provides sufficient vitamins and minerals, and multivitamins and supplements are not necessary or effective. Do you agree or disagree that multivitamins are helpful for balanced nutrition?

Keep your questions focused

Ask about only one idea at a time and avoid double-barrelled questions. Double-barrelled questions ask about more than one item at a time, which can confuse respondents.

This question could be difficult to answer for respondents who feel strongly about the right to clean drinking water but not high-speed internet. They might only answer about the topic they feel passionate about or provide a neutral answer instead – but neither of these options capture their true answers.

Instead, you should ask two separate questions to gauge respondents’ opinions.

Do you agree or disagree that the government should be responsible for providing high-speed internet to everyone?

You can organise the questions logically, with a clear progression from simple to complex. Alternatively, you can randomise the question order between respondents.

Logical flow

Using a logical flow to your question order means starting with simple questions, such as behavioural or opinion questions, and ending with more complex, sensitive, or controversial questions.

The question order that you use can significantly affect the responses by priming them in specific directions. Question order effects, or context effects, occur when earlier questions influence the responses to later questions, reducing the validity of your questionnaire.

While demographic questions are usually unaffected by order effects, questions about opinions and attitudes are more susceptible to them.

  • How knowledgeable are you about Joe Biden’s executive orders in his first 100 days?
  • Are you satisfied or dissatisfied with the way Joe Biden is managing the economy?
  • Do you approve or disapprove of the way Joe Biden is handling his job as president?

It’s important to minimise order effects because they can be a source of systematic error or bias in your study.

Randomisation

Randomisation involves presenting individual respondents with the same questionnaire but with different question orders.

When you use randomisation, order effects will be minimised in your dataset. But a randomised order may also make it harder for respondents to process your questionnaire. Some questions may need more cognitive effort, while others are easier to answer, so a random order could require more time or mental capacity for respondents to switch between questions.

Follow this step-by-step guide to design your questionnaire.

Step 1: Define your goals and objectives

The first step of designing a questionnaire is determining your aims.

  • What topics or experiences are you studying?
  • What specifically do you want to find out?
  • Is a self-report questionnaire an appropriate tool for investigating this topic?

Once you’ve specified your research aims, you can operationalise your variables of interest into questionnaire items. Operationalising concepts means turning them from abstract ideas into concrete measurements. Every question needs to address a defined need and have a clear purpose.

Step 2: Use questions that are suitable for your sample

Create appropriate questions by taking the perspective of your respondents. Consider their language proficiency and available time and energy when designing your questionnaire.

  • Are the respondents familiar with the language and terms used in your questions?
  • Would any of the questions insult, confuse, or embarrass them?
  • Do the response items for any closed-ended questions capture all possible answers?
  • Are the response items mutually exclusive?
  • Do the respondents have time to respond to open-ended questions?

Consider all possible options for responses to closed-ended questions. From a respondent’s perspective, a lack of response options reflecting their point of view or true answer may make them feel alienated or excluded. In turn, they’ll become disengaged or inattentive to the rest of the questionnaire.

Step 3: Decide on your questionnaire length and question order

Once you have your questions, make sure that the length and order of your questions are appropriate for your sample.

If respondents are not being incentivised or compensated, keep your questionnaire short and easy to answer. Otherwise, your sample may be biased with only highly motivated respondents completing the questionnaire.

Decide on your question order based on your aims and resources. Use a logical flow if your respondents have limited time or if you cannot randomise questions. Randomising questions helps you avoid bias, but it can take more complex statistical analysis to interpret your data.

Step 4: Pretest your questionnaire

When you have a complete list of questions, you’ll need to pretest it to make sure what you’re asking is always clear and unambiguous. Pretesting helps you catch any errors or points of confusion before performing your study.

Ask friends, classmates, or members of your target audience to complete your questionnaire using the same method you’ll use for your research. Find out if any questions were particularly difficult to answer or if the directions were unclear or inconsistent, and make changes as necessary.

If you have the resources, running a pilot study will help you test the validity and reliability of your questionnaire. A pilot study is a practice run of the full study, and it includes sampling, data collection , and analysis.

You can find out whether your procedures are unfeasible or susceptible to bias and make changes in time, but you can’t test a hypothesis with this type of study because it’s usually statistically underpowered .

This Pew Research Center analysis covers Israeli attitudes on the Israel-Hamas war, including opinions on how it’s being conducted, the country’s future, Israeli political leaders and the United States’ role in the conflict.

The data is from a survey of 1,001 Israeli adults conducted face-to-face from March 3 to April 4, 2024. Interviews were conducted in Hebrew and Arabic, and the survey is representative of the adult population ages 18 and older, excluding those in East Jerusalem and non-sanctioned outposts. (The survey also did not cover the West Bank or Gaza.) The survey included an oversample of Arabs in Israel. It was subsequently weighted to be representative of the Israeli adult population with the following variables: gender by ethnicity, age by ethnicity, education, region, urbanicity and probability of selection of respondent.

Here are the questions used for the report, along with responses, and the survey methodology .

A new Pew Research Center survey finds that 39% of Israelis say Israel’s military response against Hamas in Gaza has been about right, while 34% say it has not gone far enough and 19% think it has gone too far.

According to the survey, conducted in March and early April, roughly two-thirds of Israelis are also confident that Israel will either probably (27%) or definitely (40%) achieve its goals in the war against Hamas. Still, majorities of Israeli adults are worried about aspects of the ongoing war: 

  • 61% say they are extremely or very concerned about the war expanding into other countries in the region.
  • 68% say they are extremely or very concerned about the war going on for a long time.

When it comes to what should happen after the war, there is less consensus. A 40% plurality of Israelis think Israel should govern the Gaza Strip. Smaller shares think Gazans should decide who governs (14%) or would like to see a Palestinian Authority national unity government either with (6%) or without (12%) President Mahmoud Abbas (also known as Abu Mazen) in leadership.

Separately, 26% of Israelis think a way can be found for Israel and an independent Palestinian state to coexist peacefully with each other – down from  35% who said the same last year , prior to the war, and about half as many as took that position when the question was first asked in 2013.

Research in the West Bank and Gaza

Pew Research Center has polled the Palestinian territories in previous years, but we were unable to conduct fieldwork in Gaza or the West Bank for our March/April 2024 survey due to security concerns. We are actively investigating possibilities for both qualitative and quantitative research on public opinion in the region and hope to be able to provide more data in the coming months.

These are among the key findings of a new survey of 1,001 Israelis, conducted via face-to-face interviews from March 3 to April 4, 2024.

The survey also asked Israelis about the U.S. role in the conflict. (It was conducted before U.S. President Joe Biden took a tougher stance toward Israel in the wake of an Israeli airstrike that killed seven World Central Kitchen aid workers. And it predates Biden’s declaration that the U.S. would not provide offensive weapons to Israel in the event of a Rafah invasion as well as the subsequent Israeli strikes in Rafah .)

The survey shows:

  • 60% of Israelis disapprove of the way Biden is handling the Israel-Hamas war.
  • 41% think Biden is striking the right balance between Israelis and Palestinians. Still, 27% of Israelis say he is favoring Israelis too much, while roughly the same share (25%) say he favors Palestinians too much.
  • Most Israelis express confidence in Biden to handle world affairs and have a favorable view of the U.S. But ratings of both Biden and the U.S. have fallen at least 10 percentage points since last year. (For more on this, read “How Israelis and Americans view one another and the U.S. role in the Israel-Hamas war.” )

Nonetheless, a large majority (72%) still want the U.S. to play a major role in diplomatically resolving the war – more than say the same about any of the other countries or organizations asked about, including Egypt (45%), Saudi Arabia (29%), Qatar (27%) and the United Nations (24%).

Arab and Jewish Israelis

People across Israeli society perceive the war in vastly different ways, depending on their views of the current leadership, how they identify ideologically, their religious backgrounds and other factors. One of the starkest divides is between Arab and Jewish Israelis:

  • Arab Israelis are less likely than Jewish Israelis to think Israel will succeed in achieving its war aims (38% vs. 76%) and less optimistic when thinking about the future of the country’s national security (21% vs. 63%).
  • Israeli Arabs are much more likely than Jews to say the country’s military response has gone too far (74% vs. 4%).
  • Almost no Israeli Arabs (3%) want Israel to govern the Gaza Strip after the war, while half of Israeli Jews think it should do so. A plurality of Arabs would like the people who live in Gaza to decide who governs (37%), while only 8% of Jews prefer this outcome.
  • Arab Israelis have much less favorable views of the U.S. than Jewish Israelis do (29% vs. 90%), as well as less confidence in Biden (21% vs. 66%). They are also much more likely to disapprove of Biden’s handling of the war (86% vs. 53%) and to think he favors Israelis too much (86% vs. 11%).
  • Although a majority of Arabs (63%) want the U.S. to play a major role in diplomatically resolving the war between Israel and Hamas, an even greater share of Jewish Israelis (74%) want this. And roughly two-thirds of Arabs are open to Qatar and Egypt playing a major role, while only about four-in-ten Jews or fewer say the same.
  • Roughly nine-in-ten Arabs (92%) have a negative view of Israeli Prime Minister Benjamin Netanyahu, compared with around half of Jews (48%). Views of the two other war cabinet members , Benny Gantz and Yoav Gallant, are also divided along ethnic lines. (The survey was conducted before Gantz threatened to leave the war cabinet .)

In many cases, there are also large ideological differences, with Israelis who describe themselves as being on the left generally more critical of Israel’s war response, less optimistic about its success and more critical of the U.S. than those on the right. There also tend to be differences among Israeli Jews based on how religiously observant they are. For more on how we looked at these differences, refer to the box below.

Jewish religious groups in Israel: Haredim, Datiim, Masortim and Hilonim

Nearly all Israeli Jews identify as either Haredi (commonly translated as “ultra-Orthodox”), Dati (“religious”), Masorti (“traditional”) or Hiloni (“secular”). The spectrum of religious observance in Israel – on which Haredim are generally the most religious and Hilonim the least – does not always line up perfectly with Israel’s political spectrum. On some issues, including those pertaining to religion in public life, there is a clear overlap: Haredim are furthest to the right, and Hilonim are furthest to the left, with Datiim and Masortim in between. But on other political issues, including those related to the Israeli-Palestinian conflict and views of the United States, differences between religious groups do not always mirror those between people at different points on the ideological spectrum. Because of sample size considerations, we combine Haredim and Datiim for analysis in this report.

Epidurals may do more than relieve pain—they could save lives

New research links epidurals to a 35 percent lower risk of serious complications from childbirth, including blood clotting and sepsis. But doctors say it’s still okay to opt out.

With childbirth being a leading cause of death in many areas of the world , expectant mothers have long tried to mitigate the fear and discomfort associated with having a baby. Historically, that’s meant using all manner of   pain-relief interventions including hypnosis, opium, counterpressure, water immersion, and herbal remedies such as raspberry leaf and black cohosh.

Many women still use some of these options, but there are more choices today including pain-relieving narcotics, specialized massage therapy, acupuncture, and nitrous oxide. The most popular choice: receiving an injection in the lower back, known as an epidural.

“While there are a variety of other methods of pain relief available, nothing is as effective as the epidural,” says William Camann, a physician and director emeritus of obstetric anesthesiology at Brigham and Women’s Hospital in Boston.

Though pain relief is the primary reason most women choose an epidural, new research from scientists at the University of Glasgow and the University of Bristol shows that having an epidural also lowers the risk of serious complications in the weeks following childbirth—by as much as 35 percent.

“This finding underscores the importance of providing women with accurate information about epidurals and ensuring all women have equal access to them,” says Rachel Kearns, lead author of the study and an honorary professor at the University of Glasgow Medical School in Scotland.

The study also shows an epidural-related 50 percent risk reduction for women in complicated pregnancies and a 47 percent risk reduction among mothers delivering preterm.

For Hungry Minds

“These are statistically very significant beneficial effects” for at-risk expectant mothers choosing to receive an epidural, says Mark Zakowski, chair of the American Society of Anesthesiologists’ committee on obstetric anesthesia, who was not involved in the study.

The research also addresses “a very important issue in maternal health, which is keeping pregnant people safe during childbirth,” adds Yalda Afshar, a maternal-fetal medicine specialist and the co-director of the cardio-obstetrics program at UCLA Health in California, who was also not involved in the research.

But the study has limitations, and its researchers only focused on birth complications related to the mother, not the newborn child. Some of these studied maternal complications included blood clotting, infections requiring hospitalization such as sepsis, excessive bleeding, hysterectomy, and risk of heart failure.

The researchers noted these and other major complications occurring during the six weeks following childbirth among 567,216 women who had babies in Scotland from 2007 through 2019.

While these results are encouraging for this population, because 93 percent of the women included in the research were white and living within one country, "it limits (the study’s) applicability to more ethnically diverse populations," Kearns notes.

Why epidurals are helpful

Among the reasons epidurals are thought to mitigate complications is that having one can alleviate the mother's stress response, "such as lowering stress hormone levels, blood pressure, and heart rate," explains Kearns. It can also ameliorate the physiological demands of cardiovascular and respiratory work, says Zakowski, which can provide "important physiologic protections to people with underlying and perhaps undiagnosed medical conditions."

In case of emergencies, epidurals also have the advantage of being localized and a safer alternative to general anesthesia—if the epidural was already in place when the emergency began.

"Since labor is unpredictable, if you need an emergency C-section, having an epidural will prevent complications or harm to mother or baby that’s sometimes associated with general anesthesia," says Meleen Chuang, chief of obstetrics and gynecology at NYU Langone Hospital in Brooklyn.

Epidurals can also provide an opportunity for mothers in especially long labors to rest and regain their strength to be able to potentially avoid a cesarean section altogether, "due to improved tolerance of a multi-hour or multi-day excruciating experience," says David Gutman, an obstetric anesthesiologist at the MUSC Health University Medical Center in South Carolina.

In this way, an epidural may also provide a less traumatic experience for laboring mothers, which has been linked to decreased instances of postpartum depression.

Access to robust medical care

While epidurals can reduce complications, this new research also underscores the importance of access to competent medical care for more populations around the world.

"Although it is possible that the epidural medications studied in this research improved outcomes by reducing stress, I think it's more likely that having an anesthesia provider become part of the care team was the magic dust here," says Philip Hess, a physician and anesthesiology researcher at Harvard Medical School, who was not involved in the research.

He points to the extra pair of eyes and critical-care training that anesthesia providers receive as being especially helpful; plus being at a medical care center capable of providing epidurals usually means the presence of additional diagnostic and monitoring equipment that can prevent or catch many complications.

Kearns agrees that these external factors likely significantly improved the results of her teams’ research and adds that laboring women getting epidurals are also more likely to receive additional hydration and medication intravenously than mothers laboring without a dedicated injection site.

"From our data, we can’t fully separate the direct effect of the epidural from the package of care it involves as having an epidural during labor changes the woman's care pathway to enhance the ability to manage adverse events," she says.

Lingering questions

Alexandra Grosvenor Eller, a maternal-fetal medicine physician at Intermountain Health in Salt Lake City, also points to the fact that the study’s postpartum period of six weeks instead of the more commonly studied period of two to four weeks could also complicate the study’s findings.

"Other studies typically focus on complications immediately encountered during the delivery hospitalization," she explains. "Postpartum serious morbidity is complex, and while some later cases could plausibly be associated with labor and the presence of an epidural in ways we don't understand very well, many may not be related."

Gutman says there are also "innumerable confounding factors when evaluating on so broad a scale," as medical practices can vary from hospital to hospital and region to region. "It is therefore hard to extrapolate the population data from Scotland to, let's say, Charleston, South Carolina," he says.

Indeed, a JAMA study from 2022 showed a significantly more modest 14 percent reduction in complications stemming from epidural use, and that study similarly measured the data of more than half a million women—though in New York hospitals instead of Scottish ones.

If the new research results can be replicated in another country, however, it will be encouraging.

"Pregnancy is not without risk, and this is even more true for minority populations," says Elizabeth Mack, a physician and the head of the pediatric critical care unit at MUSC Children’s Health in South Carolina. She notes that the maternal mortality rate in 2022 in the United States was 22.3 deaths per 100,000 births , while that number was 13.4 deaths per 100,000 births in the United Kingdom. "Therefore, equitable adoption of factors that will mitigate birth-related risks such as epidural anesthesia are important to consider," she says.

Not getting an epidural is still okay

While epidurals are among the safest and most proven medical interventions available today, some of the reasons expectant mothers sometimes choose not to get them is a fear of needles, not wanting to "miss out" on the full birth experience, their high price tag—without insurance, the added cost of an epidural can range from $1,000 to $8,000—or religious or cultural beliefs, notes Zakowski.

"The sensation of numbness is also very unusual, and some women do not like it," adds Chuang. "One can also get what's called a 'spinal headache' after the epidural, which may be difficult to treat," says Mack. She adds that, in rare cases, nerve damage can also occur.

Regardless of one's reason for choosing not to get an epidural, Camann says there are "other methods of pain relief available—both pharmacological and nonpharmacological." Hess agrees that choosing whether to get an epidural for pain relief or not is "100 percent an individual decision," which healthcare providers should not try to influence unless preexisting conditions need to be considered.

"Women with low-risk pregnancies should feel empowered to make the choice that best suits their personal preference," Eller echoes. "I think it's premature to suggest that all women be strongly recommended to get an epidural to reduce severe maternal morbidity."

Afshar similarly stresses that the new research "in no way states that not choosing to have a labor epidural is unsafe;" rather, she hopes the results will reassure families choosing to get one that the procedure is effective, safe, and, in some cases, “may even reduce one's risk of serious complications.”

Understanding and Evaluating Survey Research

A variety of methodologic approaches exist for individuals interested in conducting research. Selection of a research approach depends on a number of factors, including the purpose of the research, the type of research questions to be answered, and the availability of resources. The purpose of this article is to describe survey research as one approach to the conduct of research so that the reader can critically evaluate the appropriateness of the conclusions from studies employing survey research.

SURVEY RESEARCH

Survey research is defined as "the collection of information from a sample of individuals through their responses to questions" ( Check & Schutt, 2012, p. 160 ). This type of research allows for a variety of methods to recruit participants, collect data, and utilize various methods of instrumentation. Survey research can use quantitative research strategies (e.g., using questionnaires with numerically rated items), qualitative research strategies (e.g., using open-ended questions), or both strategies (i.e., mixed methods). As it is often used to describe and explore human behavior, surveys are therefore frequently used in social and psychological research ( Singleton & Straits, 2009 ).

Information has been obtained from individuals and groups through the use of survey research for decades. It can range from asking a few targeted questions of individuals on a street corner to obtain information related to behaviors and preferences, to a more rigorous study using multiple valid and reliable instruments. Common examples of less rigorous surveys include marketing or political surveys of consumer patterns and public opinion polls.

Survey research has historically included large population-based data collection. The primary purpose of this type of survey research was to obtain information describing characteristics of a large sample of individuals of interest relatively quickly. Large census surveys obtaining information reflecting demographic and personal characteristics and consumer feedback surveys are prime examples. These surveys were often provided through the mail and were intended to describe demographic characteristics of individuals or obtain opinions on which to base programs or products for a population or group.

More recently, survey research has developed into a rigorous approach to research, with scientifically tested strategies detailing who to include (representative sample), what and how to distribute (survey method), and when to initiate the survey and follow up with nonresponders (reducing nonresponse error), in order to ensure a high-quality research process and outcome. Currently, the term "survey" can reflect a range of research aims, sampling and recruitment strategies, data collection instruments, and methods of survey administration.

Given this range of options in the conduct of survey research, it is imperative for the consumer/reader of survey research to understand the potential for bias in survey research as well as the tested techniques for reducing bias, in order to draw appropriate conclusions about the information reported in this manner. Common types of error in research, along with the sources of error and strategies for reducing error as described throughout this article, are summarized in the Table .

Sources of Error in Survey Research and Strategies to Reduce Error

The goal of sampling strategies in survey research is to obtain a sufficient sample that is representative of the population of interest. It is often not feasible to collect data from an entire population of interest (e.g., all individuals with lung cancer); therefore, a subset of the population or sample is used to estimate the population responses (e.g., individuals with lung cancer currently receiving treatment). A large random sample increases the likelihood that the responses from the sample will accurately reflect the entire population. In order to accurately draw conclusions about the population, the sample must include individuals with characteristics similar to the population.

It is therefore necessary to correctly identify the population of interest (e.g., individuals with lung cancer currently receiving treatment vs. all individuals with lung cancer). The sample will ideally include individuals who reflect the intended population in terms of all characteristics of the population (e.g., sex, socioeconomic characteristics, symptom experience) and contain a similar distribution of individuals with those characteristics. As discussed by Mady Stovall beginning on page 162, Fujimori et al. ( 2014 ), for example, were interested in the population of oncologists. The authors obtained a sample of oncologists from two hospitals in Japan. These participants may or may not have similar characteristics to all oncologists in Japan.

Participant recruitment strategies can affect the adequacy and representativeness of the sample obtained. Using diverse recruitment strategies can help improve the size of the sample and help ensure adequate coverage of the intended population. For example, if a survey researcher intends to obtain a sample of individuals with breast cancer representative of all individuals with breast cancer in the United States, the researcher would want to use recruitment strategies that would recruit both women and men, individuals from rural and urban settings, individuals receiving and not receiving active treatment, and so on. Because of the difficulty in obtaining samples representative of a large population, researchers may focus the population of interest to a subset of individuals (e.g., women with stage III or IV breast cancer). Large census surveys require extremely large samples to adequately represent the characteristics of the population because they are intended to represent the entire population.

DATA COLLECTION METHODS

Survey research may use a variety of data collection methods with the most common being questionnaires and interviews. Questionnaires may be self-administered or administered by a professional, may be administered individually or in a group, and typically include a series of items reflecting the research aims. Questionnaires may include demographic questions in addition to valid and reliable research instruments ( Costanzo, Stawski, Ryff, Coe, & Almeida, 2012 ; DuBenske et al., 2014 ; Ponto, Ellington, Mellon, & Beck, 2010 ). It is helpful to the reader when authors describe the contents of the survey questionnaire so that the reader can interpret and evaluate the potential for errors of validity (e.g., items or instruments that do not measure what they are intended to measure) and reliability (e.g., items or instruments that do not measure a construct consistently). Helpful examples of articles that describe the survey instruments exist in the literature ( Buerhaus et al., 2012 ).

Questionnaires may be in paper form and mailed to participants, delivered in an electronic format via email or an Internet-based program such as SurveyMonkey, or a combination of both, giving the participant the option to choose which method is preferred ( Ponto et al., 2010 ). Using a combination of methods of survey administration can help to ensure better sample coverage (i.e., all individuals in the population having a chance of inclusion in the sample) therefore reducing coverage error ( Dillman, Smyth, & Christian, 2014 ; Singleton & Straits, 2009 ). For example, if a researcher were to only use an Internet-delivered questionnaire, individuals without access to a computer would be excluded from participation. Self-administered mailed, group, or Internet-based questionnaires are relatively low cost and practical for a large sample ( Check & Schutt, 2012 ).

Dillman et al. ( 2014 ) have described and tested a tailored design method for survey research. Improving the visual appeal and graphics of surveys by using a font size appropriate for the respondents, ordering items logically without creating unintended response bias, and arranging items clearly on each page can increase the response rate to electronic questionnaires. Attending to these and other issues in electronic questionnaires can help reduce measurement error (i.e., lack of validity or reliability) and help ensure a better response rate.

Conducting interviews is another approach to data collection used in survey research. Interviews may be conducted by phone, computer, or in person and have the benefit of visually identifying the nonverbal response(s) of the interviewee and subsequently being able to clarify the intended question. An interviewer can use probing comments to obtain more information about a question or topic and can request clarification of an unclear response ( Singleton & Straits, 2009 ). Interviews can be costly and time intensive, and therefore are relatively impractical for large samples.

Some authors advocate for using mixed methods for survey research when no one method is adequate to address the planned research aims, to reduce the potential for measurement and non-response error, and to better tailor the study methods to the intended sample ( Dillman et al., 2014 ; Singleton & Straits, 2009 ). For example, a mixed methods survey research approach may begin with distributing a questionnaire and following up with telephone interviews to clarify unclear survey responses ( Singleton & Straits, 2009 ). Mixed methods might also be used when visual or auditory deficits preclude an individual from completing a questionnaire or participating in an interview.

FUJIMORI ET AL.: SURVEY RESEARCH

Fujimori et al. ( 2014 ) described the use of survey research in a study of the effect of communication skills training for oncologists on oncologist and patient outcomes (e.g., oncologist’s performance and confidence and patient’s distress, satisfaction, and trust). A sample of 30 oncologists from two hospitals was obtained and though the authors provided a power analysis concluding an adequate number of oncologist participants to detect differences between baseline and follow-up scores, the conclusions of the study may not be generalizable to a broader population of oncologists. Oncologists were randomized to either an intervention group (i.e., communication skills training) or a control group (i.e., no training).

Fujimori et al. ( 2014 ) chose a quantitative approach to collect data from oncologist and patient participants regarding the study outcome variables. Self-report numeric ratings were used to measure oncologist confidence and patient distress, satisfaction, and trust. Oncologist confidence was measured using two instruments each using 10-point Likert rating scales. The Hospital Anxiety and Depression Scale (HADS) was used to measure patient distress and has demonstrated validity and reliability in a number of populations including individuals with cancer ( Bjelland, Dahl, Haug, & Neckelmann, 2002 ). Patient satisfaction and trust were measured using 0 to 10 numeric rating scales. Numeric observer ratings were used to measure oncologist performance of communication skills based on a videotaped interaction with a standardized patient. Participants completed the same questionnaires at baseline and follow-up.

The authors clearly describe what data were collected from all participants. Providing additional information about the manner in which questionnaires were distributed (i.e., electronic, mail), the setting in which data were collected (e.g., home, clinic), and the design of the survey instruments (e.g., visual appeal, format, content, arrangement of items) would assist the reader in drawing conclusions about the potential for measurement and nonresponse error. The authors describe conducting a follow-up phone call or mail inquiry for nonresponders, using the Dillman et al. ( 2014 ) tailored design for survey research follow-up may have reduced nonresponse error.

CONCLUSIONS

Survey research is a useful and legitimate approach to research that has clear benefits in helping to describe and explore variables and constructs of interest. Survey research, like all research, has the potential for a variety of sources of error, but several strategies exist to reduce the potential for error. Advanced practitioners aware of the potential sources of error and strategies to improve survey research can better determine how and whether the conclusions from a survey research study apply to practice.
